    Is it possible to limit the quantity of images in the cart to just 1 if the image is marked as downloadable in the price list?

    In other words, the customer should not be able to check out with multiples of the same file.

    yes, you could but not through the settings you will have to add something on the cart or use the Image Store API to make the change
