bespoke cart page with extra options

hope you are well.

thanks for all your help setting me up with the cd functionality by the use of your image-store extension plugin, its working great!

i can modify the checkout pages, and would like to accomplish the following:

• add another name / address section for so i can have separate billing / delivery fields
• add a checkbox that asks the customer if the billing / delivery address is the same and disable one set of the if the checkbox is set
• display these new fields in the admin sales detail section
