try updating your price list, and try removing the $o option. if the $0 option is the problem try providing a promotion code with a 100% to purchase the images for free
do all your image have an extension on the server? I will do a full test on IE and let you know my findings
When you update the plugin, make sure that all the files are deleted and updated. The subtotal are not being calculated and the values need to be passed to PayPal.
Also it could be because their is no color option try just having the “full color ” option see if that make a difference.

no, not via shortcode you can always copy the image url and add that in the content <img src=”IMAGE-LINK” /> or user the code mentioned above.
I have seen other site using the /%postname%/ structure and it works fine I will do a full test and let you know, there where other issues on previous version you might want to try 3.1.4 and update your permalink structure and let us know if you still have the same issue.

user shouldn’t be sent directly to paypal without viewing the cart first, it my be a javascript issue on your site, could you please provide a link to your site.
